National Guest Permission Form |
Use this form when you invite guests to participate in Troop outings or other activities. It should be noted that BOTH the National form AND the Troop form (listed next) should be completed prior to participation in the activity. A Weekend Medical Form (either Youth or Adult, as appropriate) is also required. |
| Troop 317 Guest Permission Form |
This form is used IN ADDITION TO the form listed above when Troop members invite guests to participate in Troop outings or other activities. It should be noted that BOTH the National form (above) AND this Troop form should be completed prior to participation in the activity. | Adult Guest CSYP Waiver Form |
This form should be completed by any non-registered Adult who will be participating in a one-time outing or activity. Adults who intend to participate regularly should complete the Trail Life USA Adult registration process. |
